Floored Physical Therapy, LLC (d/b/a Pelvic Floored)

By purchasing, enrolling in, or participating in any online course, program, class, or digital content (“Program”) offered by Floored Physical Therapy, LLC, including its owners, employees, contractors, affiliates, and agents (collectively, “Floored Physical Therapy”), you agree to the following Terms & Conditions:

1. Educational & Informational Purposes Only

I understand and acknowledge that all Programs provided by Floored Physical Therapy are intended for educational and general health and wellness information purposes only.

The content provided is not medical advice, does not establish a provider–patient relationship, and is not a substitute for individualized evaluation, diagnosis, or treatment by a qualified healthcare professional.

2. Medical Clearance & Self-Screening

I affirm that I have been screened by a qualified medical provider and have been cleared to participate in physical activity, exercise, and/or health and wellness education of this nature.

I acknowledge that it is my responsibility to consult with a physician, physical therapist, or other qualified healthcare provider regarding any medical concerns, symptoms, diagnoses, pregnancy or postpartum status, pain, or limitations before beginning or continuing participation in any Program.

3. Assumption of Risk

I understand that all physical activity and participation in health and wellness programs carry inherent risks, including but not limited to muscle soreness, strain, aggravation of pre-existing conditions, injury, or other physical or psychological effects.

I voluntarily and knowingly assume all risks, both known and unknown, associated with my participation in any Program offered by Floored Physical Therapy.

4. Personal Responsibility

I agree to listen to my body, modify activities as needed, and discontinue participation if I experience pain, discomfort, dizziness, shortness of breath, or other concerning symptoms.

I understand that I am solely responsible for my participation, decisions, actions, and results.

5. Release of Liability

To the fullest extent permitted by law, I hereby release, waive, and discharge Floored Physical Therapy, LLC, its owners, instructors, employees, contractors, agents, and affiliates from any and all claims, demands, causes of action, damages, or liabilities arising out of or related to my participation in any Program, including but not limited to personal injury, illness, or property damage.

This release applies even if such injury or damages arise from negligence, except where prohibited by law.

6. Indemnification

I agree to indemnify and hold harmless Floored Physical Therapy from any claims, losses, liabilities, damages, or expenses (including reasonable attorney’s fees) arising from my participation in the Program or violation of these Terms & Conditions.

7. Agreement & Acceptance

By purchasing or participating in any Program offered by Floored Physical Therapy, I acknowledge that I have read, understood, and voluntarily agree to these Terms & Conditions and Liability Release.
